Essential Functions and Responsibilities:
  • Medical Management. Primary responsibility for primary care services, acute care services, routine monitoring of chronic conditions.
  • Quality Improvement. Implementing best practices to avoid hospitalizations and emergency department visits and maximize functional improvements and quality of life.
  • Team Support: Collaborate and provide clinical support to nurse practitioners, typically one per facility, empowering them with guidance and sharing medical expertise.
  • Regulatory Compliance.

Financial Rewards and Benefits:
  • Annual compensation ranges from $110,000. to $120,000., based on volume. You are in control of your earning potential.

Qualifications
  • Unrestricted professional license.
  • Ability to practice independently and complete accurate, timely documentation.
  • Experience in geriatrics, preferred.
  • Experience in post-acute care, preferred.
